Which type of gift certificate is right for you?

• Virtual Gift Certificate: This is added directly to a patient account
• Physical Gift Certificate: Your gift certificate will be created as a physical copy, the recipient will need to present the gift certificate in order to use it. At checkout you can choose to have this mailed or picked up in-office.

LIMIT 1 PER PATIENT. Please note that duplicate purchases for the same patient will be refunded. If purchasing as a gift for someone else, be sure to indicate who the recipient should be in your order note field. Promotional value expires 1 year from purchase date.

All treatment purchases will be added directly to your patient account as credits unless you request otherwise in your order notes section. Please note that your online account on our website is not the same as your account in our in-office system. Your online account will not reflect the credits on your patient account.
